We respect your privacy. Your contact information is used to contact you with respect to meetings or important chapter information. I agree to donate a minimum of $100 to a South Dundas project or charity. I understand that even if the winners are not who I would choose, I will donate regardless. If unable to attend the annual meeting personally, I will ensure my donation is received by the organizing committee, on the day of, or in advance of the meeting.

I understand that photos may be taken at meetings for use on the website and in promotion.  I give permission for my likeness to be used. 

I understand that my name will be included as a member of this group.
